We have created this privacy statement in order to demonstrate our firm commitment to privacy. The following discloses our information gathering and dissemination practices for our associated web sites.

Insofar as information collected by the Organization may be covered by the Health Insurance Portability and Accountability Act (HIPPA), all such information is maintained as Confidential in strict conformity with HIPPA rules. In such case, the person to whom such information pertains may obtain copies upon request in conformity with the requirements of HIPPA.
Information Voluntarily Provided by You
When you set up an account on our Websites or purchase an item from our Websites, we collect personal information provided by you voluntarily that may include your: (i) first and last name; (ii) billing address; (iii) shipping address; (iv) other shipping addresses; (v) email address; (vi) phone number; (vii) credit card account number; (viii) items purchased from us. How We Use Your Information
We fully cooperate with law enforcement agencies in identifying those who use our services for illegal activities and may, in our sole discretion, disclose personal information or other information to satisfy any law, regulation, subpoena, or government request. To use some features on the IFC, LLC, Website(s), a user must first complete a registration form on the Website. During registration, a user is required to give contact information, such as name and email address. This information is used to contact users about the services on our Websites for which they have indicated interest. You select your own username and password – both are needed to place an order, access personally identifiable information or edit any of your personal information.
Disclosure of Information to Third Party Advertisers and Partners
We will not rent, sell or share your personal information with third parties except as specifically approved or under the circumstances described below. If you do not want us to use or disclose personal information collected about you in the ways identified in this policy, you should not use the Websites or services.
A. Disclosure for Legal Reasons
We may disclose and release your personal information to third parties: (a) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order; or (b) in special cases, such as a physical threat to you or others, a threat to homeland security, a threat to our system or network, or cases in which we believe it is reasonably necessary to investigate or prevent harm, fraud, abuse, illegal conduct or a violation or alleged violation of this policy or other agreement we may have with you.
B. Disclosure for Shipping and Billing Reasons
We use an outside shipping company to ship orders placed on our Websites, and a credit card processing company to bill users for goods and services purchased on our Websites. These companies will not retain, share, store or use any of our user’s personal information for any secondary purposes. These parties are not allowed to use such personal information except for the purpose of providing these services.
Like most standard Websites, we use log files on our servers. These log files store information regarding IP (internet protocol) addresses, browser type, ISP (internet service provider), referring/exit pages, platform type, date/time stamp, and number of clicks. These log files are used to analyze trends, administer the Websites, track user’s movements in the aggregate, and gather broad demographic information for our use. IP addresses, etc. are not linked to personal information.

Send to a Friend
If a user elects to use our referral service to inform a friend about a part of our Websites, a form asks for the friend’s name and email address. The form will automatically send the information to the friend via email. We may store this information for tracking and reporting purposes, and we will use the form only to send this one-time referral email.
We follow generally accepted industry standards to protect the personal information submitted to us, both during transmission and once we receive it. No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore, while we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security.
Communications from our Websites
We send all new members a welcoming email to verify their password and user name. Established members will occasionally receive information on products, services, special deals, and publications. Each user has the option to not receive these types of communications (see Choice/Opt-Out below
Occasionally, it is necessary to send out service-related announcements. For instance, if our service is temporarily suspended for maintenance, we might send you an email informing you of such service interruption. Generally, users may not opt-out of receiving these service-related communications. In addition, we communicate with our users on a regular basis to provide requested services and regarding issues relating to their account via email.
Correction/Updating Personal Information
If a user’s personal information changes or if a user no longer desires our service, we will enable our users to correct, update or remove from our Websites the personal data they provided to us. Such changes can be made either through our Websites or by emailing our Webmaster at info@IntegrativeCareConsulting.com
ICC, LLC will retain your information for as long as your account is active or as needed to provide you services. If you wish to cancel your account or request that we no longer use your information to provide you services contact us at info@IntegrativeCareConsulting.com. We will retain and use your information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.
Notification of Changes in Use of Information
Protecting the privacy of the very young is especially important. For that reason, this web site never collects or maintains information at our web site from those we actually know are under the age of 18, and no part of our web site is structured to attract anyone under 18.
All web site promotions, contests and sweepstakes are open only to persons over the age of 18. No information should be submitted to or posted on this web site by persons under 18 years of age, nor should any games, contests and/or sweepstakes be played by children under 18 years of age. This web site does not knowingly collect personal information from children under the age of 18 and does not sell such information.
If we discover that we have received any information from a child under 18 in violation of this policy, we will delete that information immediately. This site has security measures in place to protect the loss, misuse and alteration of the information under our control. We use secure server technology to ensure none of your private information is intercepted by outside parties. It is the responsibility of the user to ensure their browser is capable of using this technology. We are not responsible for the results of sabotage, changed regulations and laws or other causes beyond our control.
